Activity Description

This interactive CME activity is designed to improve individual and team-based care, knowledge and skills. By participating, you will hear faculty review thought-provoking concepts that impact the optimal assessment and treatment of Type 2 Diabetes Mellitus (T2DM) so you can better develop individualized treatment plans for your patients.

Given the rising prevalence of T2DM in United States, and the challenges of coordinating care between specialists, primary care physicians, and patients, this program aims to improve care processes, interdisciplinary team function, and patient outcomes. The faculty will address and discuss the real challenges to T2DM treatment decision-making, as well as review practical strategies for overcoming barriers to care.

Target Audience

This certified enduring material has been developed for cardiologists, endocrinologists, internal medicine, general practice, family practice, nurse practitioners, nurses, certified diabetes educators, and other healthcare professionals caring for and treating people with T2DM.

Learning Objectives

Upon completion of this activity, participants will be able to:

  1. Summarize the prevalence of T2DM among special populations in the United States.
  2. Discuss relevance of optimal T2DM patient management in all patients, with a focus on those at risk and those representing racial and ethnic minority populations in Georgia.
  3. List most common barriers to providing optimal T2DM care from a population-specific perspective, as well as an urban and rural perspective, and develop strategies to overcome them.
  4. Compare ADA and AACE guidelines and explain how clinicians can apply evidence-based guidelines and algorithms to improve T2DM care in the primary care setting.
  5. Assess and determine the most appropriate treatment plans for patients using single, dual, and triple therapy regimens.
  6. Review the pros and cons for earlier use of insulin in T2DM patient management.
  7. Describe common beliefs of racial and ethnic minority populations, and how they may impact adherence to the treatment plan.
  8. Apply strategies to help you effectively communicate with patients about nutrition, diet, exercise, medication adherence and blood glucose monitoring.

AAFP accreditation begins September 18, 2012. Term of approval is for one year from this date with option for yearly renewal.

Method of Participation

There are no fees for participating and receiving CME credit for this activity. In order to receive credit for completing this activity, participants must:

  1. Read the learning objectives, accreditation information and faculty disclosures at the beginning of the activity.
  2. Register online for this activity and complete a brief pre-test;
  3. Review each of the four modules presented in this activity;
  4. Complete the online posttest and evaluation after you review each of the four modules;
  5. Physicians who receive a grade of 70% or better on the posttest and who complete the evaluation will receive a CME certificate.
  6. All other participants who receive a grade of 70% or better on the posttest and who complete the evaluation will receive a certificate of participation.
